Logan has 8 years of healthcare experience, with 5 years experience as an Administrator of Skilled Nursing Facility. She holds a Masters of Arts in Gerontology, emphasis on Healthcare Administration and Public Health. Logan became a Licensed Nursing Home Administrator in 2018.
She loves working with residents because you can create relationships with residents and their families that span several years. For skilled nursing patients she enjoys seeing them become equipped to meet their goals as a result of our care. Integrity to staff and excellent provision of quality care is the paramount focus!

Kyla has been in healthcare for 11 years, with two of those helping to manage the dietary team. She is currently working towards getting my CDM/CFPP Certified Dietary Manager/ Certified Food Protection Professional Certificate.
Kyla loves working with the residents and using their input to help build delish nutritionally balanced meals, while helping to provided them the education to make their own decisions regarding food.

Danette has been a therapist 25, 16 of those years have been at FHR in skilled nursing and 10 years as DOR. Following college graduation she spent her first "career" raising her family and working only PRN until her children went to college.
Danette loves the skilled nursing environment and working with the older residents. It allows daily opportunities to serve those who are often marginalized, feel forgotten and alone. Being a team leader also holds the responsibility of mentoring therapists and colleagues with compassion, making time to provide education, listen and be understanding, which helps keep self in check. Danette highly values facilitating others growth and helping them thrive at work and life.

Kasey has 14 years of experience in skilled nursing facilities, working to master the MDS for the last 10. She became RAC-CT certified receiving RN in 2011. She enjoys teaching CNA classes in her down time.
Kasey enjoy working as an MDS nurse because it is like a puzzle- put all the pieces together and see how it helps create the resident’s plan of care while they are here with us.

Diane has six and a half years of healthcare experience and has been a Certified Life Enrichment Director since 2023. Diane loves what she does everyday.
She gets to take care of the residents in so many ways in their lives here. Diane values Activities, relationships, and building trust with residents and their families. She has loved her experience here, it humbles her in so many ways.

Jadyn is the Director of Social Services and a dedicated Geriatric Social Worker with extensive experience supporting older adults and their families, specializing in assessing complex needs, navigating care transitions, and advocating for quality of life through personalized care plans. She is passionate about promoting independence, dignity, and well-being in the later stages of life while collaborating with healthcare teams and community resources to address the unique challenges of aging.

Sandy has been in Healthcare for 11 years. She got her CNA in 2013, Activity Director's certification in 2017 and CMA in 2019.
Sandy is enthusiastic, playful and fun. She chose this profession because she is very compassionate and loves making people laugh. Sandy loves seeing the smile on residents faces and loves making a difference.

Sarah has over 10 years of experience in healthcare and is an RN with an Associate of Applied Science Nursing degree, which has provided me with a strong foundation in clinical care and patient advocacy. She chose the nursing profession because she has always felt called to care for others and find fulfillment in helping people feel better. Skilled nursing is particularly meaningful to her as it allows her to empower patients with the strength and education they need to transition successfully to their home environments.
Sarah has embraced the role of Nurse House Manager here at Family Health & Rehab. She has found great satisfaction in teaching and guiding employees to grow in their roles and provide the best care possible.

Lilly a Social Service Designee with many years of experience working with older adults. She is dedicated to providing comprehensive care coordination and advocacy for older adults and their families, specializing in navigating complex medical needs, coordinating home care services, and developing personalized care plans to optimize quality of life while ensuring access to appropriate resources and support systems.
