Our Services

Our clinicians provide proactive, exercise oriented therapy with manual techniques, empowering patients to successfully progress through treatment and continue to improve post-rehabilitation. While staying at Family Health and Rehabilitation we will be sure to stay in touch with your primary care physician and communicate all changes in care with them.

An on site Physician Assistant offices in the building Monday through Friday 8:00 a.m. to 5:00 p.m and sees residents daily. The PA is also on call 24/7. Nursing staff is on site 24 hours a day. Nursing restorative is in place to help short and long term patients maintain their existing level of function while receiving therapy.

Outpatient therapy is available for previous residents and outpatient clients.

Our certified and professional staff are here to help you on your journey back to full health and functionality. We will work with you step-by-step to get you back to your life as quickly as possible.

  • The therapy department is open six days a week for the convenience of our residents. Treatment frequency is based on the physician’s order and the therapist’s clinical assessment.

    Our occupational therapists focus on improving a patient’s ability to perform the activities of daily living (ADL). These may include cooking, dressing, bathing, etc. Residents may also spend time working on conserving energy to make daily activities easier, improving balance, increasing strength in upper extremities, or increase fine motor control.

    Treatment may include:

    • Strengthening specific muscle groups to help restore fine and gross motor skills.

    • Comprehensive home evaluation and patient education.

    • Instruction in adaptive living techniques, joint protection, work simplification, energy conservation and motor coordination.

    • Customized adaptive equipment.

  • Our professional staff consists of certified speech-language pathologistswho are dedicated to applying innovative clinical pathways that ensure each patient receives the utmost in care, on a personalized level. Our talented Speech Therapists help you, or your loved one, work with all aspects of communication from slurred speech patterns to improving writing skills. They also work to improve a patient’s ability to swallow.

    Our speech-language pathologists treatment plans may include:

    • Improved functional communication skills and speech production

    • Improved oral feeding and swallowing skills, including vital stimulation

    • Improved memory and thinking or establishes compensatory strategies in these areas

    • Teach alternative methods to communications such as communication boards

  • Our foodservice team focuses on culture change and resident’s food choices. Each kitchen features it’s own chef and provide family style dinning for all three daily meals. Made to order food is also available any time of day. Family and friends are encouraged to come visit and join residents for meals throughout the day.

    We consult directly with a licensed dietitian who helps create meal plans for those with special dietary needs. Our care plan team reviews nutritional interventions and discusses nutritional modifications. The dietary staff then follow standardized recipes and menus created and approved by the Dietitian.

    Offering a variety of food choices and enhanced nutritional care.

    We provide healthy eating choices for customers with weight loss, diabetes, dysphagia, food allergies, tube feeding, etc. We consult directly with a licensed dietitian who helps create meal plans for those with special dietary needs. Our care plan team reviews nutritional interventions and discusses nutritional modifications. The dietary staff then follow standardized recipes and menus created and approved by the Dietitian.

    In addition, the foodservice staff is trained in food safety, sanitation procedures, and quality standards in food production to ensure all food is prepared properly.

    Our dedicated staff of registered physical therapists, certified physical therapy assistants and their support team will work with you to improve the areas that will make the most difference in your life. Each resident is different and it is our goal to create a plan that fits your needs and will help get you back to the life you deserve to live.

    Comprehensive Care Plans

    Our physical therapists develop comprehensive plans of care that may include:

    • Exercises to restore motion, strength, and a greater degree of independence

    • Joint and soft tissue mobilization

    • Pain management including use of modalities such as ultrasound and electrical stimulation

    • Orthotic/prosthetic training

    • Lymphedema management, including wraps and manual lymph drainage

    • Fall assessments to teach preventative measures in case of future falls

    • Teach back care and injury prevention

    • Increase ability to walk distances, with or without a supportive device

  • Our nursing staff is comprised of RNs, LPNs, CNAs, and CMAs. Each of our houses have an RN or LPN on staff twenty-four hours a day 7 days a week. We have the latest in technology including medical records technologies that keep all records safe and secure on our computer system. We utilize safe and efficient guidelines and systems to ensure your care is the highest quality possible.

  • Item descriOrthopedic Rehabilitation – Therapy’s focus is on preventing contractures, increasing range of motion and strength, and instructing patients on precautions, exercises, and adaptive equipment needs. Therapy strives to return an individual to their prior level of function and living environment.

    • Neurological Care – The therapy team of OTs, PTs, and STs work together to facilitate muscle use and control. Various techniques are utilized including manual therapy, weight bearing activities, vital stimulation, and electrical stimulation.

    • Pain Management – Therapy works to assess location of pain, pain intensity, and identify best pain management techniques. Techniques may include: electrical stimulation, moist heat, ultra sound and manual therapy.
